Nevada Apartment Deposit Agreement

Mark-Taylor Residential, (“Lessor”), and (“Applicant”), hereby agrees as follows:

1. Applicant has made written application to Lessor to lease Apartment No. at the Apartments, located at .

2. Applicant understands that they will not be permitted to occupy the premises or execute a rental agreement for the premises unless and until Lessor has approved the Applicant’s credit references and has confirmed, based upon Applicants application, that Applicant has the ability to pay the monthly rent for the apartment.

3. In the event that Applicant is approved by Lessor, Lessor shall notify Applicant by telephone. In the event Lessor notifies Applicant he/she is approved and Applicant fails to execute the proposed Lease Agreement, Lessor may retain the Non-Refundable Fee and the Application Fee for liquidated damages. In any event, the Non-Refundable Fee and Application Fee becomes non-refundable after 48 hours from the time this agreement is signed by the applicant.

Applicant understands that their electric and/or gas utility account number is/are required at time of move-in. Applicant also understands that they are responsible for the payment of the following utilities:

As a condition of your lease, our community requires the following:

1. Residents must maintain and provide proof of coverage for a minimum of in legal liability protection prior to move in. This is to protect the property and resident from damage to the Landlord's property to include damage caused by Water, Fire, Smoke and Explosion.

2. Our Community requires all insurance carriers to list - P.O. Box 498067 Cincinnati, OH 45249 as an "Interested Party" on the Declaration page and that the insurance company will notify the community in the event of a cancellation or change in policy status.

3. Water coverage must be provided for negligent resident-caused damage to the insured's unit of possession as well as adjacent units damaged due to the insured's negligent actions. (i.e. If I accidentally overflow my bathtub, will this policy cover the damages to my unit and adjacent units?)

Security Deposit Addendum

UNIT #:

The parties to the Apartment Lease hereby enter into the following Security Deposit Addendum (hereinafter "Addendum"). Where provisions of this Addendum conflict with those set forth in the Apartment Lease, the provisions set forth herein shall supersede and be controlling.

Resident and Management have agreed that Resident has the obligation to provide Management with a security deposit at the commencement of the term of the Lease. Resident understands that security deposits, in addition to guarantying full and faithful performance of the lease by the Resident, protect Management and the owner of the community from property damage or other losses.

The opportunity to purchase security deposit insurance and renter's insurance from Management's preferred provider partner is available to all residents as an option to lower Resident's up-front move in costs. Resident is not required to purchase security deposit insurance and, if Resident chooses to not purchase both the security deposit insurance and renter's insurance from ePremium Insurance LLC, Resident agrees to deliver the security deposit to Management at the commencement of the term of the Apartment Lease in certified funds.

Resident has been presented two options regarding the security deposit: payment of the traditional security deposit or acquisition of a policy of Security Deposit Insurance through ePremium Insurance LLC, described below.

Resident hereby elects the following option with regard to the Security Deposit:

A. Traditional Security Deposit. Resident shall deliver to Management funds equal to the amount identified on the line "Security Deposit" in the third column ("Refundable Deposits") under the Monthly Rent, Non-Refundable Fees, and Refundable Deposits section of the Apartment Lease. Resident is aware that Resident must still obtain and maintain a policy of renter's insurance.

B. ePremium Security Deposit Insurance. Resident has purchased a policy of security deposit insurance and a policy of renter's insurance from ePremium Insurance LLC. Resident understands and agrees that maintaining these policies of insurance (and the payment of all monthly premiums in a timely manner) is a material obligation under the Apartment Lease.

If Option "A" is selected, the numbered and lettered paragraphs set forth below are deemed stricken from this Security Deposit Addendum and shall not be applicable. If Option "B" is selected, the numbered and lettered paragraphs set forth below apply to this Security Deposit Addendum.

1. By electing Option "B," Resident has chosen to reduce Resident's upfront move-in cost through the acquisition of two insurance policies through ePremium Insurance LLC: security deposit insurance and renter's insurance. Resident understands that the security deposit insurance policy is available only when accompanied by and bundled with a policy of renter's insurance from ePremium Insurance LLC:.

2. In the event that coverage under either policy lapses through cancellation, discontinuation, or any other reason, Resident shall immediately deliver to Management, in certified funds, a security deposit in the amount of

3. Resident accepts, understands, and agrees that Resident shall be responsible for the payment of all monetary sums for which Resident became liable due to Resident's conduct or the conduct of Resident's invitees or guests to the community.

5. Management agrees to release Resident from responsibility only for damages where payments are made under one or both above-referenced policies of insurance.

6. Resident has chosen to purchase an insurance product. The money paid to the insurance company is not a security deposit and is not refundable. Even if Management makes no claim against the insurance policy, Resident will not be entitled to any refund of the insurance premium at the end of Resident's possession of the leased premises.

7. Resident understands and agrees that no claim shall be made against the policies of insurance for unpaid rent or utilities charges. Resident further agrees that any rent, community transfer fee, early termination fee, late fees, legal fees or utilities charges left unpaid at the end of the period of Resident's possession shall remain Resident's sole responsibility.

NOTICE: ePremium Insurance LLC is not owned or operated by Management or Owner, and Management makes no guarantees or representations concerning the insurance and/or the services ePremium Insurance LLC provides. Furthermore, Resident acknowledges that Resident has chosen to purchase a product of insurance at Resident's own free will and that Resident is under no obligation to purchase security deposit insurance.
